If the trial is already completed, please submit the following:
- The Clinical Trial Status Report
- The Final Clinical Study Report/ Synopsis/ Publication
Please submit the both of the reports online at prism@hsa if the initial CTC application was submitted online.
For CTC applications that were submitted manually, please download the Clinical Trial Status Report Form.
If the Final / Clinical Study Report / Synopsis / Publication is not available yet, please notify us in writing as to when it will be available.
