Application for Certificate of Registration or Certificate of Licence
Please refer to the
fee schedule for the fees and charges for Certificates.
ONLY fee payment by GIRO shall be accepted.
A GIRO account and a Client Registration and Identification System (CRIS) account with HSA shall have to be set-up prior to payment via GIRO.

To apply for a Certificate of Registration or Certificate of Licence, please send in your request to
HSA_MD_INFO@hsa.gov.sg.
Information to be submitted as part of email application
1. Email Subject Heading to specify either one of the following:
a. Application of a Certificate of Registration, or
b. Application of a Certificate of Licence
2. The approved Device Registration Number or approved Licence Number
3. The applicant's company Client Code
4. The applicant's information as reflected in MEDICS
Applicant will be informed of the application outcome by email.
