Licence Fee is per year and is payable at the point of submission of the application. Cheque payment shall be crossed and made payable to "Health Sciences Authority". Applicant must indicate his/her customer code and application number on the overleaf of their cheque. Subsequent payment of licence should be made by GIRO.
