As part of our continual effort to streamline the administrative processes for the Form C Poisons Licences, we wish to inform you that with immediate effect, pharmacists rotating among the various pharmacy outlets registered under the same company are no longer required to submit amendment application to amend the pharmacy outlet and warehouse particulars reflected in their Form C Poisons Licences. However, this arrangement will not be applicable if the pharmacist is transferred to another outlet due to a closure of the pharmacy outlet where the pharmacist is licensed under. In such cases, the pharmacist will still have to submit an amendment application to ensure that he/she is holding a Form C Poisons Licence reflecting the particulars of a valid pharmacy.

Please take note that the submission of an amendment application of the Certificate of Registration of a Pharmacy is still necessary if the rotation of pharmacist(s) among the various outlets involve changes of the pharmacist-in-charge assigned for a pharmacy outlet.
