Safety concerns associated with design limitations of Graseby Syringe Drivers (MS16A and MS26)

HSA, in consultation with its Medical Device Advisory Committee, would like to alert healthcare professionals of design limitations of Graseby Syringe Drivers (Models MS16A and MS26) and incidents related to medication delivery of these devices reported globally. The incidents include incorrect amounts of fluids being delivered (either more or less than what had been programmed) and the device stopping during infusion. These incidents occurred without any audible alarms. Healthcare professionals who are still using Graseby Syringe Drivers MS16A and MS26 are advised to use the devices with caution, and repair and maintain them appropriately. Other registered alternative devices should be considered given the design limitations of the affected devices, cessation of their manufacture, and the limited service and repair support provided by Smith Medical till 2019.
