Failure of Receipt
Failure of Receipt is defined as not receiving a First Schedule Part 2, Second Schedule, or Fifth Schedule biological agent or toxin, whether as a transfer from another facility or as an import, within 24 hours of the time reasonably estimated by the recipient.
Requirements
Notification of failure of receipt to the Health Sciences Authority (HSA) is required for First Schedule Part 2, Second Schedule and Fifth Schedule biological agents and toxins, and is strongly encouraged for First Schedule Part 1 biological agents.
Notification
If the transferee (recipient) failed to receive the delivery of the biological agent/toxin within 24 hours of such time as may be reasonably estimated by him/her, he/she must inform the HSA by calling or sending an e-mail on the failure of receipt of the biological agent/toxin.
The recipient is also required to login to the Healthcare Application and Licensing Portal-Biosafety Module (HALP-BS (opens in new tab)) to do the notification online.
