Issue during reconstitution with Actilyse® (alteplase) powder and solvent for solution for injection and infusion

Boehringer Ingelheim International GmbH would like to highlight a recent issue of the rubber stoppers being pushed into the vial during the reconstitution of Actilyse® Treatment Set 50mg/vial. Over the last months, Boehringer Ingelheim has received an increased number of overseas complaints for Actilyse® 50 mg describing the rubber stoppers being pushed into the vial during reconstitution. This makes the vial unusable. Healthcare professionals are advised to follow the reconstitution process stated in guidance document enclosed with the letter.
